About the department

The Department of Obstetrics and Gynecology of the Medical Faculty of the Higher Women's Courses was organized in Moscow in 1909.

At the Department of Obstetrics and Gynecology of the Faculty of Medicine under the leadership of Professor Yu.E. Dobrokhotova is actively conducting pedagogical and scientific work. More than 500 students, 25-30 residents, and 10-12 graduate students study at the department annually. The department pays special attention to the training of professional personnel. The entire teaching staff of the department works at medical centers. They actively participate in the treatment process: they supervise patients, are on duty, and participate in advisory examinations of patients who especially deserve attention.

The Department of Obstetrics, Gynecology and Reproductive Medicine of the Faculty of Advanced Training of Medical Workers of the RUDN University was formed in 1996, one of the first at the faculty, on the basis of the staff of the Department of Obstetrics and Gynecology with the course of perinatology of the RUDN University. The basis for the creation of the department of postgraduate education was the high pedagogical authority of the scientific school, embodying the unity of science and education in Russia, headed by the Honored Scientist of the Russian Federation, Professor Viktor Evseevich Radzinsky. Under his leadership, the scientific and pedagogical staff of the department continues the traditions of outstanding obstetricians and gynecologists, professors who stood at the origins of this school: Alexey Aleksandrovich Ivanov, Honored Scientist of the Russian Federation Vyacheslav Ivanovich Eltsov - Strelkov, Evgeny Vasilyevich Mareev.

The modern interpretation of the principles of clinical examination of pregnant women, based on the results of dissertation research by the department’s staff, made it possible to develop an obstetric and perinatal risk strategy that combines the quintessence of modern views of the world community and the preventive focus traditional for domestic health care. The development of the surgical direction of scientific research was accompanied by a powerful impetus for scientific research in the most promising areas of obstetric and gynecological science and practice. Currently, in the field of scientific interests of a team of like-minded people, headed by Professor V.E. Radzinsky, are many aspects of women's reproductive health.

One of the priorities of the department remains the promotion and implementation of modern perinatal technologies into the practice of medical institutions, focused primarily on the family. This is, first of all, breastfeeding, which is one of the first-order priorities of the national health care systems of most countries of the world, the joint stay of mother and child, as well as minimizing obstetric and pharmacological aggression against the pregnant woman and the fetus.

Within the framework of the implementation of the priority national projects of the Russian Federation “Education” and “Health”, the informational, educational, treatment and preventive work of the teaching staff and graduate students with adolescents and young people, the initiative of which was supported by the rector’s office and the administration of the University, received a wide public, political and scientific resonance. The successful experience of a close-knit team of enthusiasts, accumulated during the implementation of the university programs “Health” and “Reproductive Health of Students”, reflected in 2 doctoral and 2 candidate dissertations, numerous publications in Russia and abroad, has been extended to more than 35 classical and medical universities, higher and secondary specialized educational institutions of the Russian Federation and Ukraine.

In the last decade, during the period of globalization and erasing of intellectual boundaries, the achievements of the scientific and pedagogical staff of the department have received unconditional recognition in Russia and countries near and far abroad. Professors and teachers are permanent members of the teams of authors of national and international guidelines on obstetrics and gynecology, are members of expert groups of the Ministry of Health of the Russian Federation participating in the development of procedures for providing care and clinical protocols for the management of pregnant women and gynecological patients, and actively present the results of their work at Russian and foreign conferences. Close scientific and creative ties have been established and are successfully developing with teams from research institutes and universities in the Russian Federation, Ukraine, Kazakhstan, and Israel, which provide postgraduate training for specialists in the field of obstetrics, gynecology and reproductive medicine.

The traditional all-Russian and international forums “Early Pregnancy”, “Breast Medicine”, “Infections and Infection Control”, “Cervix and Vulvovaginal Diseases” (Moscow) and “Reproductive Potential of Russia” (Sochi, Kazan) are rightfully included to the list of events most attended by specialists in the field of obstetrics, gynecology, perinatology, fundamental and applied reproductive medicine from the Russian Federation and around the world.

The Department of Obstetrics and Gynecology of the Perm State Medical Institute was founded in 1920 by Professor Evgeniy Mitrofanovich Kurdinovsky on the basis of the maternity and gynecological departments of the Perm Provincial Zemstvo Hospital. A brilliant erudite and excellent lecturer, he was known for his physiological and pharmacological experiments on an isolated uterus, a course of lectures on the pathological histology of the female genital area, as well as the first major monograph in Russia, “Analgesia of Normal Childbirth.” During these years, clinical and histological laboratories were created, and the creation of a museum of the department began.
Since 1926, the department was headed by Professor Alexander Fedorovich Palmov. He is the author of the world-famous work “Therapy of Birth with a Narrow Pelvis”, as well as studies on high forceps and placenta previa. Under his leadership, the surgical activities of the clinic are significantly intensified, and vaginal methods of operative gynecology are introduced into practice.
Since 1932, the department has been headed by a prominent scientist, talented organizer, Professor Pavel Alekseevich Guzikov. The obstetric department increased to 80 beds, the gynecological department - to 60 (in 1920 there were 10 obstetric beds, 40 gynecological beds). Under the leadership of Professor Guzikov, 9 candidate and 2 doctoral dissertations were defended. Employees of the department and clinic actively participate in the work of the Scientific Society of Obstetricians and Gynecologists, created in 1935. Professor Guzikov P.A. was its first chairman. Since 1952, Professor Nikolai Pavlovich Lebedev, a prominent scientist, one of the authors of a multi-volume manual on obstetrics and gynecology, has been elected head of the department. His arrival coincides with the restructuring of teaching at the institute in connection with the introduction of 6-year education. The educational bases of the department are expanding, including maternity hospitals in the city of Perm. The department is developing and introducing into healthcare practice cesarean section techniques, a comprehensive method for preventing bleeding during childbirth, and psychoprophylactic preparation of a pregnant woman for childbirth.
Since 1956, the department has been headed by Associate Professor Nikolai Konstantinovich Nakaryakov, known as a brilliant gynecologist surgeon. He developed methods for the surgical treatment of purulent lesions of the uterine appendages and the surgical treatment of vesicovaginal fistulas. N.K. Nakaryakov was awarded the title “Honored Doctor of the RSFSR”, his name was included in the Institute’s Book of Honor.
Since 1959, the head of the department was Associate Professor Vladimir Sergeevich Soloviev, an excellent clinician and experienced surgeon. He introduced vaginal surgical methods into practice. Since 1971, the head of the department has been Associate Professor Roza Gavrilovna Skachedub, an experienced clinician, talented lecturer and teacher, whose erudition, charm, and excellent organizational skills contributed to the enormous pedagogical, medical and advisory work carried out by the department.
Since 1974, the department has been headed by Professor Georgy Tarasovich Ishchenko. 3 candidate's theses were defended. Improved radiological research methods are being widely introduced into practice. Tactics for the management of patients with purulent tumors of the uterine appendages, fibroids and endometriosis are being developed. In 1986, a course of obstetrics and gynecology was created at the faculty of advanced training for doctors (since 1986 - an independent department), which is headed by an employee of the department of obstetrics and gynecology of the medical faculty, associate professor Galina Sergeevna Leutkina.

Since 1992, the department has been headed by Associate Professor German Andreevich Kozin. He was the president of the first created association of obstetricians and gynecologists of the Perm region. The department continues to work on the problem of “Prevention of maternal and perinatal morbidity and mortality.” During the difficult years of perestroika, new methods for diagnosing the condition of the mother and fetus are being introduced, therapeutic approaches for infertility, dystrophic diseases of the vulva, and anomalies of labor are being improved. 3 candidate's theses were defended.
Since 2000, the department was headed by Professor Boris Evgenievich Grebenkin, who was also the director of the Perinatal Center, created on the basis of the maternity ward of the Regional Clinical Hospital. The scientific activities of the department are devoted to the prevention of pregnancy complications. Under the guidance of Professor B.E. Grebenkin, 13 candidate's theses and 1 doctoral dissertation were defended. Methods of surgical approaches in obstetrics and gynecology are being improved. Endoscopic methods of diagnosis and treatment are being actively introduced. In the intensive care unit for pregnant women, programs for the management of pregnant women with severe gestosis are being developed.

In 2013, the Department of Obstetrics and Gynecology of the Faculty of Medicine merged with the Department of the Faculty of Pediatrics.

The department participates in the organizational and methodological work of medical institutions in the Perm region; is the organizer of annual scientific and practical conferences and educational round tables for doctors of the city and region. The Department of Obstetrics and Gynecology was organized at the pediatric faculty of the 2nd Moscow Regional State Medical Institute named after. N.I. Pirogov in 1951 by Honored Scientist of the RSFSR Professor Anatoly Alekseevich Lebedev. This was the first department of obstetrics and gynecology of the pediatric faculty in the USSR.

The department is constantly improving methods of managing pregnancy and childbirth in uncomplicated and complicated cases: breech presentation, untimely rupture of amniotic fluid, postmaturity, chronic fetal hypoxia, intrauterine growth retardation. According to the results of the competition of the Russian Foundation for Basic Research, the department was recognized as the leading scientific school of the Russian Federation. In 2000, Professors G.M. Savelyeva, L.G. Sichinava, R.I. Shalina, P.A. Klimenko, M.A. Kurtser were awarded the Moscow Mayor’s Prize for a series of works on the topic “Ways to reduce perinatal morbidity and mortality by optimizing the management of pregnancy and childbirth."

In the field of gynecology, the department's staff is intensively developing the following topical issues: physiology and pathology of postmenopause, infertility, inflammatory diseases of the genitals. A revolutionary stage in the development of scientific research in this direction was the development of endoscopic methods for the diagnosis and treatment of gynecological diseases (laparoscopic, hysteroscopy). For the first time, laparoscopy and hysteroscopy began to be performed in our country in 1977 at one of the department’s bases - in the City Clinical Hospital No. 31.

The first results of diagnostic endoscopy with elements of some surgical procedures were reflected in the monograph “Endoscopy in Gynecology,” published under the editorship of G.M. Savelyeva in 1983. Hysteroscopy, as a new method for studying the inner surface of the uterus, has created a new direction in the study of intrauterine pathology. A large section of the department’s work was devoted to the study of endometrial pathology during the postmenopausal period: pathogenesis, clinical picture, diagnosis and therapy of these conditions; the principles of management of patients with bleeding in the postmenopausal period against the background of endometrial atrophy have been determined.

The department introduced and modernized various hysteroscopy techniques. The arsenal of hysteroscopic operations performed has expanded significantly with the introduction of electrosurgical techniques. The department's employees were among the first to use operative hysteroscopy to treat patients with recurrent endometrial hyperplasia. The methods and scope of surgical interventions are constantly being improved. Currently, hysteroscopy is widely used to remove polyps, myomatous nodes, dissection of intrauterine septums, and adhesions. The methods of uterine endosurgery developed at the department made it possible to avoid transection and removal of the uterus, and contributed to the preservation of reproductive and menstrual function. Modern methods of treating intrauterine pathology such as hysteroresection, laser and thermal ablation have been introduced.

The clinic has accumulated extensive experience in the use of laparoscopy for gynecological diseases requiring diagnosis and surgical treatment on an emergency and planned basis. Laparoscopy was initially carried out mainly for diagnostic purposes, and surgery consisted of “minor” operations and manipulations: ovarian biopsy, coagulation of a bleeding lesion, adhesiolysis. Subsequently, the department began to perform operations on the uterine appendages: elimination of the tubo-peritoneal factor of infertility, creation of artificial obstruction of the tubes for the purpose of contraception, for ectopic pregnancy, ovarian formations, and in 1994 the first hysterectomy was performed. In recent years, hysterectomy performed laparoscopically has become routine.

Currently, all gynecological operations that can be performed using laparoscopic access are not only being introduced into clinical practice in the department’s clinics, but are also constantly being improved, their feasibility is confirmed by scientific research, and long-term results after surgical interventions are studied.

Laparoscopic surgery for acute inflammatory diseases of the uterine appendages deserves special attention. The work carried out at the department has shown that acute inflammatory processes in the abdominal cavity not only do not serve as a contraindication to laparoscopy, but often determine the indications for it. It is difficult to overestimate the importance of the dynamic laparoscopy developed and put into practice by the staff of the department, which made it possible to obtain objective data on the gradual regression of signs of inflammation, the presence of adhesions in the pelvis, and makes it possible to timely determine the indications for transection.

Scientific research carried out at the department made it possible to prove that the number and nature of surgical interventions on the abdominal organs in the past is not a contraindication to laparoscopy. In order to increase safety during laparoscopic entry into the abdominal cavity in these patients, alternative points for introducing the first trocar, as well as open laparoscopy, have been developed.

To the employees of the department, Academician of the Russian Academy of Medical Sciences G.M. Savelyeva, professors V.G. Breusenko, L.M. Kappusheva, Doctor of Medical Sciences O.V. Aziev, Doctor of Medical Sciences S.V. In 2003, Shtyrov was awarded the Russian Government Prize for the development and implementation of endoscopic methods of diagnosis and treatment in gynecology.

The department conducts fundamental research on the problem of “Physiology and pathology of postmenopause”. An algorithm for the examination and management of postmenopausal patients with endometrial pathology has been developed, and careful treatment methods have been introduced in somatically burdened patients in the presence of contraindications for hormonal therapy.

An important scientific direction of the department is the problem of diagnosis and treatment of ovarian formations. The introduction of modern diagnostic methods made it possible to create a protocol for the management of patients with this pathology, clearly define the indications for laparotomy and laparoscopy, and an adequate scope of surgical treatment. A grant was received to carry out scientific research on the topic “Early diagnosis of ovarian cancer.”

The clinic is improving and widely using ultrasound methods for diagnosing gynecological diseases - color Doppler mapping, hydrosonography.

Issues of etiology, pathogenesis, clinical picture, diagnosis and treatment of inflammatory diseases of the internal genital organs are one of the important problems being developed at the department. Methods of surgical treatment of uterine prolapse have been optimized; mesh implants are used. To treat stress urinary incontinence in women, surgical treatment methods have been introduced into practice - TVT operations. Since 2004, a modern organ-preserving method of treating uterine fibroids has been carried out - uterine artery embolization.

The department's staff is intensively developing topical issues of pediatric gynecology: the formation of the reproductive system of the female body at various stages of its maturation, menstrual dysfunction in girls, tumors and tumor-like formations of the ovaries, disorders of the timing of sexual development (premature and delayed), inflammatory diseases of the genital organs, malformations of the genitals , various genetic abnormalities

Since 1997, the clinic began operating a department of assisted reproductive technologies, where all modern methods of treating infertility are used.

The results of scientific research in the main areas of obstetrics and gynecology are summarized in 35 monographs and in more than 2,500 publications in our country and abroad, 13 patents for inventions have been received. 2 reference books and 5 manuals for obstetricians and gynecologists have been published. Under the leadership of Academician of the Russian Academy of Medical Sciences G.M. Savelyeva and professors of the department, 38 doctoral and 154 candidate dissertations were completed.

For students and residents, edited by Academician of the Russian Academy of Medical Sciences G.M. Savelyeva and with the participation of professors and associate professors of the department, 2 textbooks were published: “Obstetrics” (2000) and “Gynecology” (2004).

Every year, about 960 4-5 year students of the pediatric faculty, 25-35 residents and graduate students study at the department. Since 2000, students from the Moscow Faculty of the Russian State Medical University began studying at the clinic.
The department constantly operates a student scientific circle, in which 70-80 students take an active part every year.

Social work is another area of ​​multifaceted activities of the department’s employees, headed by Academician of the Russian Academy of Medical Sciences, Professor G.M. Savelyeva.

For many years, professors of the department have been members of the editorial boards of leading journals in the field of obstetrics and gynecology in our country and abroad, members of the Academic Councils of the Russian State Medical University, MMA named after. I.M. Sechenov, Scientific Center of Agipology of the Russian Academy of Medical Sciences, Moscow State University. M.V. Lomonosov, Problem Commission on Obstetrics and Gynecology of the Russian Academy of Medical Sciences, Higher Attestation Commission of the Russian Federation, Russian Academy of Natural Sciences.

For scientific, pedagogical and social work, Academician G.M. Savelyeva was awarded the Order of “Badge of Honor” and “Friendship of Peoples” and for services to the fatherland, IV degree.
